Minor Participant Waiver

Parental / Guardian Consent, Release of Liability & Indemnity Agreement

This Parental/Guardian Consent, Release of Liability, Waiver of Claims, Assumption of Risk and Indemnity Agreement ("Agreement") is entered into by the undersigned parent or legal guardian ("Guardian") on behalf of the minor participant identified below ("Minor"), in favor of Adventure Pursuit Outfitters, its owners, officers, employees, guides, agents, successors, and assigns (collectively, "APO").

1. Description of Activity

Guardian wishes to enroll Minor in guided fishing trips and related outdoor recreational activities operated by APO on Lake Texoma and surrounding waters in Texas and Oklahoma (the "Activity"). Guardian acknowledges that the Activity involves inherent risks including, but not limited to: exposure to natural elements and weather, operation of watercraft, use of fishing equipment and sharp implements, slippery or uneven surfaces, wildlife encounters, physical exertion, and the possibility of serious injury or death. Guardian confirms that Minor will be accompanied by a responsible adult at all times during the Activity.

2. Parental Consent

Guardian hereby grants full consent for Minor to participate in the Activity and acknowledges that they have the legal authority to execute this Agreement on behalf of Minor. Guardian confirms that Minor is in good physical health and has no medical condition that would prevent safe participation. Guardian agrees that Minor will follow all instructions given by APO guides and staff at all times.

3. Assumption of Risk

Guardian, on behalf of themselves and Minor, expressly acknowledges and assumes all risks associated with the Activity, whether foreseeable or unforeseeable, including risks arising from the negligence of APO or its staff.

4. Release of Liability

In consideration of Minor being permitted to participate in the Activity, Guardian, on behalf of themselves, Minor, and their respective heirs, executors, administrators, and assigns, hereby releases, waives, discharges, and covenants not to sue APO from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, injury, or death that may be sustained by Minor while participating in the Activity, whether caused by the negligence of APO or otherwise.

5. Indemnification

Guardian agrees to indemnify, defend, and hold harmless APO from any loss, liability, damage, or cost APO may incur due to Minor's participation in the Activity or Guardian's breach of this Agreement.

6. Medical Authorization

In the event of an emergency in which Guardian cannot be reached, Guardian authorizes APO staff to seek and consent to emergency medical treatment for Minor. Guardian agrees to be responsible for all costs associated with such treatment.

7. Photography & Media

Guardian grants APO permission to photograph or video record Minor during the Activity and to use such images or footage for promotional, marketing, or educational purposes without compensation.

8. Governing Law

This Agreement shall be governed by and construed in accordance with the laws of the State of Texas. Any disputes arising hereunder shall be resolved in the courts of Texas.

9. Severability

If any provision of this Agreement is found to be unenforceable, the remaining provisions shall remain in full force and effect.
